Physical Assessment and Immunization Requirements
Requirements are the same for students from out of state and from within Connecticut. All requirements must be met and submitted prior to attending school.
Grade Entering | Physical Assessment |
Kindergarten | A complete kindergarten physical assessment must be done within the 12 months prior to the first day of kindergarten |
Grades 1-6 | A complete kindergarten physical assessment is accepted for entry |
Grade 7 | A complete 6th grade physical assessment must be done within the 15 months prior to the first day of 7th grade |
Grade 8-9 | A complete 6th grade physical assessment is accepted for entry |
Grade 10 | A complete 9th grade physical assessment must be done within the 15 months prior to the first day of 10th grade |
Grade 11-12 | A complete 10th grade physical assessment is accepted for entry |
All Grades coming from outside the USA | A complete physical assessment done by a MD, DO, APRN or PA in the USA must be done within the 12 months prior to the first day of school entry. Physical assessments done on a USA military base outside of the USA will be accepted. |
A complete physical assessment done by a MD, DO, APRN or PA MUST include the following:
Height and weight | Vision screening |
Blood pressure | Hearing screening |
Hematocrit or hemoglobin | Gross dental assessment |
Chronic Disease Assessment | TB risk screening: If high risk for TB, then a TB test (PPD) needs to be done and read by the primary care provider before entering school |
The physical assessment and immunizations MUST be approved by the school nurse prior to school entry.
